Expose the ledger read-only to agents over stdio, TCP or Streamable HTTP.

All tools go through the read-only Runtime, except two review tools that append a review event, one gated live-query tool (off by default) and the opt-in session-memory note tool, which writes only its inbox file. --workspace resolves that directory's ekos.toml when --config is not given. Precedence: --config, then EKOS_CONFIG, then the workspace's ekos.toml, then EKOS_WORKSPACE.
