Human Review

Uncertain conclusions are stored as unconfirmed claims that a person (or reviewing agent) confirms or rejects.

EKOS prefers a reviewable question to a confident mistake.

Confirming or rejecting writes a new event to the append-only ledger; nothing is overwritten, so a decision can itself be audited or superseded. These two are the only write-capable MCP tools; every other tool is read-only.
