Versioning

Append-only history, point-in-time state and two notions of time.

Time	Meaning	Where
Observation time	when the ledger learned something	`written_at`; used by `as_of`, `ekos diff`, `ekos_state at`
Domain time	when something was true	relationship validity window (`valid_from` / `valid_until`)

Writing a changed object appends a new entry; commit skips objects identical to those already stored.
ekos diff --from A --to B lists objects and relationships written in (A, B].
ekos_architecture_diff and ekos_architecture_drift give semantic diffs (technologies, roles, risks) rather than raw entry ids.
ekos branch manages ledger branches.

Format versions. The on-disk storage generation is versioned separately (SQLite v1/v2, fact engine v3) — see Upgrades.
